The Anchor Families home-based (AFP - Anchor Families Preservation) program is an approach to deliver social services that focus on the entire family as an intervention to preserve the family unit and maintain youth in the home. The AFP program is a team approach involving the clinical director, program director, counselor and the home-based service provider. We offer both preservation and reunification services to families referred by the local Department of Child Services and probation offices.

Anchor Families' intensive home-based service provider carry a small caseload averaging 4 families at one time. Services are initiated immmediately upon referral and contact can occur within 24 hours of referral. Service frequency is determined by the needs of the families and range from 5 to 20 hours per week.

  • Linkage with psychiatric services and monitoring of any prescribed psychotropic medication.

  • Written monthly summaries of progress.
  • Safety planning.
